Our History |
The Chartercare Health Partners Foundation was established in 2010 as part of Chartercare Health Partners, the umbrella organization for St. Joseph Health Center, Our Lady of Fatima Hospital, and Roger Williams Medical Center.
In June of 2014, Chartercare Health Partners was purchased by Prospect Medical Holdings, a California based for-profit medical services company. As a result, Chartercare Foundation became a separate 501© nonprofit organization serving the broader needs of the RI Community. The Foundation changed its name to Chartercare Foundation and moved to its current location in February of 2014 and named Paula Iacono, former Development Officer for Chartercare Health Partners, as Executive Director. |

Patricia Wegrzyn McGreen
PRESIDENT
Patti holds a BS in Finance from Fairfield University and an MBA from Providence College. She retired in 2012, following a 24-year career at Citizens Financial Group, including positions as Chief Financial Officer for New England and the Midwest Regions, and Senior Vice President, Communications and Marketing.
During the past two decades, she has been active in the community in a number of volunteer roles, including Board President of House of Hope CDC, Warwick Country Club, and the RI Alzheimers Association; and board member and advisor for the East Greenwich Chamber of Commerce, RI Special Olympics, Perishable Theatre and the Warwick Museum of Art, among others. Since 2009, she has owned and operated Thorpe’s Wines and Spirits in East Greenwich. She and her husband Rob live in Warwick.

Shannon Shallcross, MS
VICE PRESIDENT — TREASURER
Shannon is founder of InsurTeck-DataTech, an East Greenwich-based data gathering company that leverages emerging technology to build custom health databases. Prior to that, she served as National Vice President for Client Management at Provant Health, and Assistant Vice President of Sales and Cient Services for Amica Insurance. She rose through the ranks to become one of the youngest female corporate officers in Amica’s history. Shannon received a BA from Dickinson College in Pennsylvania and a Masters from Boston University.
She currently serves as an appointee to the RI Department of Health’s HIE Advisory Commission and as a member of the RI Philhamonic Orchestra and Music School Board of Directors. Shannon resides in East Greenwich with her husband and two sons.

Peter F. DeBlasio, Jr., MD
SECRETARY
Dr. DeBlasio has been Chief of Ophthalmology at Our Lady of Fatima Hospital since 1996; he is also on the medical staffs of Miriam and Rhode Island Hospitals. In addition to his medical studies at the University of Bologna, he was awarded a glaucoma fellowship at the Harvard Medical School Mass Eye and Ear Infirmary.
A true renaissance man, he is also a professional jazz musician (with a bachelors from the Boston Conservatory of Music and two CDs); an inventor (with three surgical instrument patents); a National Karate Champion and Instructor; and is fluent in Chinese and Italian. He and his wife and daughter live in Providence.

Angella Franklin
DIRECTOR
Angella began her career as a pharmacy intern and has since held a variety of roles at CVS Health which have exposed her to many different areas of the business. Her past experiences in the retail division include: Pharmacist, Pharmacy Supervisor, Pharmacy Operations Manager, Regional Director of Pharmacy Acquisition, and Regional Director of Real Estate.
Angella is currently a member of the Board of Governors for Leadership Rhode Island as well as the Chartercare Foundation. Angella has a B.S. in Pharmacy from the University of Rhode Island; she and her family live in Warwick.

Carolyn Young, MD
DIRECTOR
Consultant and former Vice President and Chief Medical Officer of the RI Blood Center, Carolyn began her work there in 1995 and led the Histocompatility Laboratory to accreditation. Dr. Young’s research has resulted in the RI Blood Center being the first in the country to accept donors with tattoos; her criteria become the new AABB standard in the United States.
Dr. Young received her BS from Florida State University, her medical degree from University of Florida, and completed her residency at Brown University, where she is now a Clinical Assistant Professor of Pathology and Laboratory Medicine. She is double board certified in Clinical Pathology and in Blood Banking/Transfusion Medicine. She resides in Cranston with her husband and two children.

Carmen Diaz-Jusino
DIRECTOR
Carmen Diaz-Jusino, has over 20 years’ experience in organizational management. In addition to her work as Strategic Initiative Officer for the Rhode Island Foundation, she held various positions at the Center for Women and Enterprise, where she provided leadership across New England, raising philanthropic resources toward its mission. She has also taught business classes for family child care workers in English and Spanish, and collaborated with the Small Business Administration and community partners to identify opportunities for Latina and minority women.
She has been a tireless volunteer for many nonprofits, including serving as Chairperson of the Latina Leadership Institute of RI; Commissioner for the RI Commission on Women; Board Member and Chair of the Economic Progress Institute, and as a member of the Board of the RI State Council on the Arts. She has been awarded many accolades, including Latino Enterprise Award, Extraordinary Women Award, and the RI Dominican Independence Award for Community Leadership Service. She resides in Providence.

Mary P. Blanchet, MSN, ANCP
DIRECTOR
A graduate of the University of Indianapolis, Mary's nursing career has included stints in the Coronary Intensive Care Unit at Indiana University Medical Center; Pediatric Oncology at Riley Children's Hospital; and Nurse Practitioner at Family Care of Indiana among others. She has also served as a clinical preceptor for the Nurse Practitioner MSN Program at Indiana University. Upon moving to RI, she served at the Blackstone Valley Community Health Center and the RI College Student Health Center.
She holds certifications from the American Nurses Credentialing Center for Adult Nurse Practitioner and Family Nurse Practitioner; and she has been recognized for nursing excellence by the State of Indiana. Her academic and practical nursing experiences will be a welcome addition to the Foundation's scholarship process.

Rilwan J. Feyisitan, Jr.
DIRECTOR
Rilwan K. Feyisitan, Jr. has over 20 years of not for profit and government experience in Human Services delivery systems. He has extensive experience in working with communities of color, low-income families, refugees, seniors and members of the LGBTQQ and disability populations. He is currently President & CEO of East Bay Community Action Program, an organization that operates, promotes and supports the development of human service programs aimed at improving the lives of low-to-moderate income individuals and families living in Providence County.
His past experience includes Chief Administrator, Office of Minority Health for the RI Department of Health. He is a graduate of Roger Williams University and a certified youth sports coach. Rilwan is very involved with nonprofit agencies throughout RI, serving on a number of Boards in addition to the Foundation, including the RI Free Clinic and Norwell Leadership Academy.

Mary C. Sullivan, Phd, RN, FAAN
DIRECTOR
Dr. Sullivan is currently professor emeritus at the University of Rhode Island Nursing Education Center. She received her undergraduate degree from Salve Regina College, and her Masters in Nursing at the University of Nebraska. Her doctorate work was completed at URI. In addition to her many research honors and accolades, she was named the 2018 Nurse Scientist of the Year in RI Monthly.
She is Board certified in Massachusetts, Vermont, Nebraska, and RI. Her research focuses on Neonatal Nursing, Child Neurology and Behavioral & Developmental Pediatrics. With her academic and research background, she is a welcome addition to the Chartercare Foundation Board. Her input on behavioral health issues and our scholarship process will be invaluable.

Paula Iacono
EXECUTIVE DIRECTOR
Paula was named Executive Director of the Foundation at its inception in 2014. She has been in fundraising and friend-raising for most of her career, most recently as Development Officer for CharterCare Health Partners. She previously served her alma mater Bryant University as Director of Alumni Relations, and Johnson & Wales University as Regional Alumni Director for All Campuses.
Her community involvement includes the Advisory Council of the Little Sisters of the Poor, Pawtucket; and past service on the Board of Directors of the Brain Injury Association of RI and Children's Wishes. She is also a member of the Board of Directors for Devereaux Advanced Behavioral Health of Massachusetts and Rhode Island.
